There are two disability models used to identify persons with disabilities. First is the scientific view which locates the disability within the individual. It is either biomedical or functional. The second one is social pathology which has something to do with the
environment and human rights.

The purpose of this study is to produce an assessment tool and start the evaluation process which will contribute to the effectiveness and efficiency of Kauswagan Center in carrying its task as a service provider.
